چکیده

the aim of most pharmacotherapy for Parkinson’s disease (PD). The theoretical background to CDS is that the physiological striatal dopaminergic stimulation is constant and that oral therapy with levodopa in PD can replace dopaminergic stimulation only in an intermittent, non-physiological manner.1,2 This pulsatile stimulation of the dopamine receptors is thought to contribute to the development of motor fluctuations, which are common after a few years of levodopa treatment and may substantially affect quality of life.3,4 Oral levodopa is highly effective but cannot be delivered in a controlled way because of its short plasma half-life and its erratic absorption due to slowed gastric emptying.5 Dopamine agonists for oral administration have longer plasma half-lives than levodopa, thereby causing less pulsatile stimulation. However, the efficacy of the agonists is only partial compared with levodopa, and side effects are more common. Levodopa is therefore almost always needed in advanced PD.

Jejunal Infusion of Levodopa–Carbidopa Intestinal Gel Versus Oral Administration of Levodopa–Carbidopa Tablets in Japanese Subjects with Advanced Parkinson’s Disease: Pharmacokinetics and Pilot Efficacy and Safety

BACKGROUND AND OBJECTIVE Oral levodopa-carbidopa (LC-oral) treatment in advanced Parkinson's disease (PD) is associated with motor complications due to large fluctuations in levodopa plasma concentrations. Levodopa-carbidopa intestinal gel (LCIG) provides individualized continuous levodopa-carbidopa delivery through intrajejunal infusion. This study evaluated the pharmacokinetics, safety, and e...

Population pharmacokinetics of levodopa in subjects with advanced Parkinson's disease: levodopa-carbidopa intestinal gel infusion vs. oral tablets

AIMS Levodopa-carbidopa intestinal gel (LCIG) provides continuous levodopa-carbidopa delivery through intrajejunal infusion. This study characterized the population pharmacokinetics of levodopa following a 16 h jejunal infusion of LCIG or frequent oral administration of levodopa-carbidopa tablets (LC-oral) in subjects with advanced Parkinson's disease (PD). METHODS A non-linear mixed-effects ...

Continuous intestinal infusion of levodopa/carbidopa in advanced Parkinson's disease: efficacy, safety and patient selection.

Long-term oral therapy with levodopa is associated with the development of motor fluctuations and dyskinesia in a large percentage of patients with Parkinson's disease (PD). Motor complications are associated with a number of non-motor symptoms and have a negative impact on disability and quality of life. There are three therapeutic options available for the management of patients at this advanc...

Use of advanced therapies for Parkinson's disease in Norway.

BACKGROUND Many patients with Parkinson’s disease with severe motor fluctuations benefit from advanced therapies – either deep brain stimulation or continuous infusion therapy with levodopa-carbidopa intestinal gel or apomorphine. In Norway, deep brain stimulation is provided as a shared national or multi-regional service. The treatment is currently available at Oslo University Hospital and St....

When the going gets tough: how to select patients with Parkinson's disease for advanced therapies.

Levodopa-induced motor complications of Parkinson's disease, including motor fluctuations and dyskinesias, become increasingly frequent as the disease progresses, and are often disabling. Oral and transdermal therapies have limited efficacy in controlling these problems. Advanced device-aided therapies, including continuous infusion of apomorphine, deep brain stimulation and levodopa-carbidopa ...
